Private Pay Rates

Direct Pay:  If you do not have insurance or wish to pay direct, a siding scale fee based on total household income is available.  The minimum fee for an individual session using this payment method is 40.00 up to a maximum of 120.00.

Rates

$175 per Individual Mental Health Assessments for adults

$225 per individual Sports/Rehabilitation Counseling Assessments for athletes & Rehab patients

$90 per 45-50 minute individual session for adults

$125 per 45-50 minute individual session for athletes & rehab patients

$150 per 45-50 minute individual session for couples and families

One Time Initial Assessment:

$175.00 for up to 90 minutes for individual Mental Health Assessments for adults

One Time Initial Assessment for Athletes:

$225.00 for up to 90 minutes for sports/rehab counseling Assessments for athletes & Rehab patients

Individual Sessions (Mental Health Counseling):

$120.00 for up to 50 minutes

Individual Sessions (Sports/Rehabilitation Counseling):

$150.00 for up to 50 minutes

Group session:

$30.00 per session/per person

Swann Wellness Group is an approved provider for Blue Cross Blue Shield: Blue ChoiceNetwork.  Although we are not a provider for all insurances, you may be able to submit a claim directly to your insurance company and could be partially reimbursed for services.  Contact your insurance company customer service in advance for policy benefits. We are a fee-for-service private practice counseling agency which enables us to provide you with more flexibility, be customer driven and give greater attention to each client.
